We begin by offering a FREE consultation with one of our professional client care coordinators. Our goal is to get to know you, assess your needs, and determine the best way we can help. Our Care Coordinator will create a customized care plan that best suits your needs and finances, and then we use our Smart Match system to connect you with the best-suited caregiver. Finally, we deliver a “warm handoff” with your new caregiver, ensuring a smooth transition during the first visit, and provide regular follow-ups with our quality assurance program.

Who needs to be at the initial consultation?
During the initial, free consultation, our dedicated Client Care Coordinator will meet with you and any family members who would like to be involved. Our goal is to get to know you, understand your needs, and see how we can help.
What information is collected and what questions are asked?
The consultation will include a review of your health and medical conditions, an assessment of ADL’s (activities of daily living), a cognitive and emotional well-being assessment, and a home and safety evaluation.
How is a care plan created?
After the consultation, the Client Care Coordinator will create a customized care plan, which is then reviewed and signed by a Certified Nursing Assistant (CNA).
How is a caregiver selected for the new client?
Once a customized care plan has been established, A Better Solution uses a Smart Match System to find the best-matched caregiver for your needs.
What happens during the first visit?
During the first visit, your selected caregiver will be accompanied by our Client Care Coordinator in a “warm handoff” to ensure you and the caregiver are comfortable. The Care Coordinator will review the care plan with the caregiver to make sure everything is covered and that the care plan can be followed precisely.
Process for feedback along the way
A Better Solution In Home Care follows a 24/7/30 Quality Assurance Program. Our Client Care Coordinator will follow up with you to ensure your needs are being addressed.
24 Hours: Immediate post-discharge follow-up with clients and families to ensure a smooth transition home.
7 Days: Continuous monitoring and caregiver check-ins to ensure medications, meals, and care plans are followed.
30 Days: Comprehensive evaluation of recovery progress to prevent complications and promote long-term wellness.
